You need to book travel assessments at least 4 weeks prior to travel. If you are travelling sooner than this, you will need to find a private travel service (many pharmacies offer this).
You will need to complete a travel risk assessment form (see below) and contact the surgery at least two days later to make an appointment for your travel consultation with a nurse.
Some courses of vaccinations take two or three months to complete so please take this into consideration when planning your journey.
